Future Care Plan
One of Caring Considerations’ most popular services is our Future Care Plan. While you may not need Care Management* now, it ensures immediate support when needed in the future. Solo-agers are encouraged to consider Future Care Plan.
Our Goal: Establish a proactive Care Management relationship for future needs, with periodic communication and monitoring for crisis response, medical advocacy, holistic support, and up-to-date electronic healthcare records.
Our staff will provide periodic contact and we require updated health information twice a year.
Future Care Plan includes an annual fee for one in-person visit and one phone call. Additional fees may apply.
*What is Care Management? A Care Manager performs valuable, and professional expertise in elder care issues. Care Management helps clients maintain independence while addressing safety, health, and security. We create tailored Care Plans to prevent and respond to crises.
